Anyone running a private hospital in India has looked at HMS software at least once. A lot of you have bought one. And a lot of you found out the hard way that it didn't work the way your hospital does. The billing logic assumed something else. Lab integration needed a separate vendor. Customising the pharmacy module cost nearly as much as the software. So coordination stayed on WhatsApp, billing stayed 15 days behind, and lab results still got carried to doctors by hand.
Hospital software isn't bad, as such. Most HMS products sold in India were just designed for an imaginary average hospital, and Indian private hospitals are nowhere near average.
What most HMS products get wrong
The gaps nearly always show up in the same three places. Billing first. Most products assume a neat encounter-to-invoice flow. A typical Indian private hospital has procedures billed by department, pharmacy billed separately, insurance authorisations running alongside cash billing, and TPA rules that change from insurer to insurer. A product that struggles with the simple case handles the real one through workarounds.
Then pharmacy. In most off-the-shelf HMS products, pharmacy is either a separate module or an add-on. Dispensing isn't tied to the active patient file, so unbilled medicines fall into the gap between the ward and the billing counter. That's not a small annoyance. It's a revenue leak, and you can put a number on it.
And lab. Packaged products usually treat lab results as manual entry: the analyser prints a result, somebody types it into the HMS. Connecting the analyser straight to the patient record over HL7 or ASTM, which removes the typing and the transcription mistakes, is simply left on the table.
If pharmacy and lab aren't connected to the patient record, you don't have a hospital management system. You have a billing system with extra steps.
What a hospital in Delhi NCR actually needs
Going by the HMS we built for a 180-bed hospital in Delhi NCR, these are the requirements that really matter for a mid-size private hospital:
- OPD flow:Registration, token queue, doctor assignment and billing in a single flow, so the front desk isn't flipping between systems.
- IPD integration: Admission through discharge, tied into bed management, pharmacy, lab and billing. The discharge summary triggers the final bill. No separate process.
- Pharmacy dispensing linked to the patient file: Every item dispensed on the ward is attached to a patient record. Nothing goes out without one.
- Lab results pushed automatically:Results land in the treating doctor's patient view as soon as they're ready. No walking paper around.
- Live bed occupancy:Ward supervisors shouldn't be ringing each other to find a free bed. It should be on screen, live.
The build vs buy question
Packaged HMS looks cheaper at the start because you can see the licence fee. You can't see the customisation bill yet. Talk to any Indian private hospital that's been through an off-the-shelf rollout and you'll hear some version of the same story: customisation that cost as much as the product, an implementation partner charging ₹50,000 per change request, and a workflow that still doesn't match how the hospital runs.
